Helping Your Patients

Lowell Community Health Center’s caring professionals work together with other staff as a team to give each patient the attention, treatment and services he or she needs to ensure health and wellness. Our providers can count on various resources to take advantage of or direct patients to additional support and services, such as: 

  • Medical interpreter services:

    The ability for patients and health care providers to communicate is fundamentally important to the safety and comfort of the patient during treatment and discussion.

  • Integrated care:

    Behavioral health services are offered as part of the primary care visit, or as interventions. Routine and ongoing counseling is also offered.

  • Care management resources:

    Chronic conditions, including diabetes, hypertension, HIV, high cholesterol and asthma require education and ongoing preventive care and/or treatment.

  • Referral services:

    A key part of comprehensive care is addressing patients’ needs beyond medical treatment. We offer connections to food, housing and employment assistance.
